I have some concord vines and they are looking good - no fungus problems, no insect issues except minor problems with JBs. However, I have found several grapes that split open like the picture. The grapes are just reaching veraison. There are only a few, like one in a cluster and only every third or fourth cluster. Is this a normal occurrence or do I have a problem that needs to be corrected?

Yeah, that happens with a lot of vegetables, too. I've had tomatoes and cabbages split after a heavy rain, particularly if it is dry for awhile.
I don't know about grapes but have had the same problem with veg. The only way to avoid it is to keep the soil damp so that the Veggies etc. don't take up too much water at once.
